COURSE OVERVIEW

Join us to learn more about the Home and Community Services (HCS) programs and services that are available in a residential setting.  We will explore using the CARE Assessment to understand the client’s Plan of Care and evaluate if they are a good fit for your facility.  We will look at the client’s care team and the roles and responsibilities of their dedicated HCS Case Manager.

We will share requirements and expectations around contracts available in an Adult Residential Care, Enhanced Adult Residential Care and Assisted Living setting including solutions for both dementia and behavior challenges.  We will also provide ideas for resources and wrap around supports to allow you to be well positioned to serve the changing needs of Medicaid clients.

We hope you will join us to learn more about how HCS can help support you and your clients.
